Tim Grimes
Chief Executive Officer
Tim Grimes is the Chief Executive Officer for Concentric Health. As the CEO, Tim not only oversees Concentric Health’s strategic vision, but develops strategies and programs to help post-acute care providers, health systems, physicians and payer organizations succeed in value-based care. Tim is a thought leader and frequent speaker on bundled payments and other value based care initiatives.
Prior to joining Concentric Health, Tim has worked for over 26 years in the health care sector, specifically in hospital, physician, home care and emergency medicine environments. His focus has been on strategy and business development, partner collaboration, legal and compliance, and operations management. Tim began his career in health care working as Associate General Counsel for a 700 bed hospital in Southwest Ohio.
Tim graduated from the University of Dayton with a Bachelor of Science in Finance and earned his Juris Doctorate from the University Of Dayton School Of Law.
Julie Uhrig
Vice President of Clinical Operations and Development
Julie is the Vice President of Clinical Operations and Development for Concentric Health. Julie has been with Concentric Health from inception in 2012. Julie is responsible for the development and operational implementation of the Concentric Health care redesign strategies including care transitions, care management and data analytics. Her understanding of value based care helps ensure that the Concentric Health’s provider partners are delivering high quality care at a lower cost. Julie was able to partner with Concentric Health’s provider partners to outperform the market in the key areas of length of stay, readmission rates and 30-day cost of care.
Prior to joining Concentric Health, Julie has worked for over 30 years in the healthcare sector including hospital, homecare and skilled nursing. Most recently she was the Director of Health Care Strategic Initiatives for Life Enriching Communities which owns and operates two large senior living communities in Cincinnati, Ohio. As part of her role with Life Enriching Communities she was instrumental in the development of a memory support assisted living program and a non-medical home care services for residents. In addition to working with Concentric Health, Julie helped to develop and provides oversight to a continuing care at home program at Life Enriching Communities called Confident Living.
Julie graduated from Bethesda Hospital School of Nursing. She earned a BSN from Chamberlain College of Nursing and is currently pursuing a Master’s degree in nursing. She began her career in the acute care setting but moved to post-acute care and discovered her passion to improve the quality of services for older adults.
Julia Salzer
Care Transitions Coordinator
Julia Salzer is a Care Transitions Coordinator (“CTC”) with Concentric Health. As a CTC, Julia is an integral part of the clinical team and works hand-in-hand with post-acute providers. She ensures patients and their families are educated on medication management, community resources, and home services. She is essential in helping post-acute providers understand and navigate value-based care. This expertise leads to decreasing lengths of stay in the skilled nursing facilities while ensuring that patients transition home safely.
Julia has been a nurse since 2018 and has experience working with providers, hospitals and home health agencies. Prior to joining the team at Concentric Health, she worked as a home health Care Transitions Nurse in the hospital setting. Julia understands the importance of advocating for patients and empowering them to take charge of their own healthcare in order to achieve optimal health and wellness.
Julia graduated from Great Oaks School of Practical Nursing and attended Northern Kentucky University. Julia has an Associates of Arts and an Associates of Science from Gateway Technical and Community College.
